We believe mental & emotional health is an important part of overall health and wellness. Our behavioral health providers work closely with patients and their Primary Care Providers to help them meet their health care goals.
Job Description:
As a Case Manager, you will work directly with patients in our Integrated BH program to coordinate care and referrals to wraparound services that support clinical care. This includes enrollment into insurance and benefit programs for which clients are eligible, as well as connecting them with transportation, housing, food, behavioral health specialty care, and substance abuse treatment. The Case Manager makes proactive contact to improve patient outcomes, increase patient compliance, improve patient recall and retention, enhance patient advocacy, and develop co-operative relationships with other health and social services agencies. The Case Manager may assist with front office duties to include check-in, monitoring of patients through a metal detector, and answering incoming phone calls, as well as all other office duties that may be assigned.
Education/Knowledge/Skills:
- Associate’s or bachelor’s degree in psychology or human services
- Experience working in social services and with underserved populations
- Experience with Medicaid patients and related insurance knowledge
